Case Management Eligibility

Who is eligible for CFF Case Management Services?

In order to receive services through CFF Case Management Services, an individual must:

  • be at least 18 years old.
  • have medical documentation of an acquired brain injury.
  • be motivated to make progress towards personal and programmatic goals.
  • be willing to sign a release for obtaining necessary assessment information.
  • be willing to participate in a substance abuse evaluation and treatment if the initial client assessment performed by the Case Manager suggests a current substance problem.
  • be willing to participate in a mental health evaluation and treatment if the initial client assessment performed by the Case Manager suggests a current mental health concern.

To Apply

Individuals with a brain injury who are interested in CFF Long-Term Case Management Services can refer themselves for services or ask a professional or support person to make a referral. Referrals to the program may be received by phone, fax or mail. Upon receiving the referral, the potential client will be contacted by a Case Manager to schedule an intake.
