What is Case Management?

 

Case Management is the process of assisting a client and the client’s family or support system to identify needs and develop a plan to meet those needs. As a part of this process, a Case Manager educates the client about community resources, plans and coordinates services and, finally, monitors the services and goals that have been put in place. Case Management at CFF is client-centered and client-driven: clients create their own goals and are empowered in the case management process and improve their ability to make decisions.

 

Brain Injury Understood:

 

Life after a brain injury is often filled with complex challenges, affecting not only a survivor of brain injury, but their family and support system as well. Managing these challenges alone can be overwhelming and accessing services in the community can be confusing and frustrating. Case Managers at Community Futures Foundation provide support specific to the unique needs of survivors of brain injury, acting as an advocate for the client and family as they navigate service systems together. 

Meet our CFF Case Manager


Crystal . Lloyd, MSW, Case Manager
is a graduate of the University of South Carolina where she earned her MSW. Crystal has five years of experience as a case manager. After graduating with a BSW from James Madison University, Crystal was hired as a Case Manager for Big Brothers Big Sisters of Harrisonburg/Rockingham County. Upon entering graduate school in 2003, Crystal was offered a job as an HIV Case Manager with an AIDS Service Organization in Columbia, SC. Crystal is located at the Mill House and is providing services to people with brain injuries in the community.
